MODEL RELEASED. Suspected adverse drug reactions form. This yellow card is completed by a general practitioner to report a patient's adverse reactions (side effects) to a drug. The report is then sent to the Medicines and Healthcare products Regulatory Agency (MHRA) and the Commission on Human Medicines (CHM) who uses the information to ensure that the drug is safe to use.
